Lifebreath 267max won’t run right

Hello, just got a 267max installed and the thing won't do what it's told.  Some times it does though.  I've tried 10/50 and 20/40 and it'll either never turn off or won't turn on for hours.  If I unplug or turn off it will run right but it's hit or miss for how long. 

I have the 99-DXPL02 controller and Lifebreath confirmed it should do what it's told.  He told me though he has no clue what's wrong with it!  Thanks Lifebreath! 

Only thing I can add is the HRV is a stand alone.  No furnace or HVAC hooked up in any way!  I only have wires running from the controller R, Y, G to the units RED, YELLOW, GREEN. And the green LED on the unit turns on every 10 seconds for a solid green for 2 seconds, then off for half a second, then back on for half a second.

HVACquick.com is sending a new controller but I fear it's the mother board in the unit.  Any help?
